A general form for measuring empowerment, making a participatory appraisal, or assessing which elements of capacity have priority.

A Framework:

Use a form like this. Design it yourself to suit your requirements and conditions.

Please note that it has had to be condensed here to put onto this web site.

When you design your own form, position it as landscape (eg sideways) rather than in a portrait position as this one (it is that way only because this is a web site).

Leave more space for the notes when running a participatory evaluation session.
