1. Your U.S. Mail address used on the original registration form AND your current U.S. Mail address if different from the original.

2. The confirmation "answer" you registered to your original registration confirmation "question".

3. Your SCN email address: _______@scn.org

A reset password will be sent VIA U.S. Mail to the relevant U.S. Mail address if appropriate.
